As matter spirals onto NS or BH, it moves faster and faster (orbital velocity is greater at shorter distance). Due to violent collisions, there is actually a distribution of velocities - below and above orbital. So, small portion of moving plasma has velocity more than 1.41 of the orbital one, thus more than an escape velocity. As plasma can not easily move perpendicular to the magnetic field, it only escapes in the direction parallel to it - jetting up and down from accretion disk.
