Unfortunately, motorizing a focuser won't necessarily make it work better. You really want to tune up the focuser that you have to get rid of stiction of the mechanism. This is more important than anything else towards getting a good working focuser. I might mention putting a larger knob on for the focuser knob will also help in getting finer focus.
As to a schematic, it's so basic to switch the two wires to a motor depending upon the direction desired that I leave it to you as an exercise in the use of the mind.
