All hyperspace is is a "fifth dimension" sort of. It is purely hypothetical; it does not really exist. What it does do is allow physicists to study phenomena like gravity with more accuracy and detail than with the "standard" three- or four-dimensional models. Wormholes are much more complicated. Essentially, though, all it is is a tube that pierces the space-time continuum. it transports matter and energy passing through it instantaneously from one point to another. It is possible to go into great depth about how wormholes may be created, but that's quantum physics, which is real, REAL tough.
