Black holes do form. The observations for star stuff falling in from a collapsing star eventually disappear from any possible detection. I wouldn`t say that time slows down near the horizon though. I`d say that clocks so. If I send pulses, spaced 1 second apart, to an observer located just outside the hole than I would reckon the pulses to get there at an interval of 1 second. The local observer co-located with the clock would mention something totally different (shorter rate < 1 second).

But if you yourself fell in then (assumign the hole was massive enough) then you would measure a finite time to cross the event horizon.
