I need to know where the matter that goes into a black hole comes out at?

Why must it come out anywhere? The hole just gets bigger and bigger. Some galaxies show evidence of Black Holes that are millions of solar masses, from 'eating' surrounding stellar material. Mechanism that powers Quasars.

Also do black holes ever die off?

Its thought they do. Hawking radiation implies the hole looses mass, spin and charge. Given enough time the hole simply evaporates.
