Easy. Your talking about local structure such as stars, galaxies and maybe clusters and groups. Cosmologically we are homogeneous and isotropic on scale much larger that that. The reason we talk about large structure is because it is a observable fact and secondly because variation in this homogeniety and isotropism is a result of the quantum nature of our universe. During inflation variation occurred due to quantum fluctuations which were spread over very great distances. Distances isomorphic to the scale your talking about but not distances great enough to effect the large scale structure of our homgeneous and isotropic universe.
